TASK 2: How You Navigate the Screen The screenshot below shows an example of an Account Maintenance screen. When you open an Account Maintenance screen, what do you typically do first?(Required) Scan through multiple sections to get an overview of the account Go directly to one specific section to view or edit information It varies depending on why I opened the record If you go directly to one section, which section is it most often? TASK 3: Finding the Account Type The screenshot below shows the Account Information section. The Account Type field (highlighted) indicates whether the record is a Facility, Owner, or Personal/Business account. How important is it to see the account type (Facility, Owner, Personal/Business) immediately when you open an account record?(Required) Very important—I need to confirm the account type right away Somewhat important—I check it occasionally Not important—I rarely need to verify account type Is the account type easy to find in its current location, or would you prefer it to be displayed more prominently (for example, at the very top of the screen)?(Required) The current placement works well for me I would prefer it to be more prominent I’m not sure TASK 4: Where to Find the Edit Option To keep the interface consistent, edit options will appear in the same location throughout the Account Maintenance screen. We’re deciding between two approaches and would like your input. Option A places a single Edit link in the upper-right corner of each section. Clicking it opens the entire section for editing. Option B places Edit links next to editable groups of information. Clicking one opens just that specific item for editing. If you needed to update information like a phone number or address, which approach would work better for how you use the system?(Required) Option A: A single Edit link for the entire section Option B: Edit links next to each individual item/group Either approach would work for me TASK 5: Field Names Across States Some fields, like "Corporation" or "Organization," may have different official names depending on your state. Which approach would help you work more efficiently?(Required) Use my state’s official terminology, even if it differs from other states Use one consistent term across all states Either approach would work for me Are there specific field names that cause confusion? TASK 6: Reviewing Internal Notes Internal Notes are the dated entries that inspectors and staff add to an account record over time—observations, follow-ups, or other information about the business. The screenshot below shows how these notes currently appear. When reviewing Internal Notes for an account, what information helps you most? (Select all that apply)(Required) Seeing the full text of each note displayed on the screen Seeing just the date and first line, with the option to expand for full text Being able to search or filter notes by keyword or date I rarely need to review past Internal Notes Select All TASK 7: Hours of Operation Display We’ve redesigned the Hours of Operation section. The screenshots below show the current design (top) and the proposed new design (bottom). (Current app) (Proposed design) Which layout makes it easier to quickly understand a facility's schedule?(Required) The current design The proposed new design Both are equally clear Neither is clear (please explain) What would make the schedule easier to understand? The new design uses colored labels to indicate whether the facility is Open, Closed, or by Appointment Only on each day. How easy or difficult is it to understand the facility's availability using the colored labels?(Required) Very easy Somewhat easy Neither easy nor difficult Somewhat difficult Very difficult What would make the status easier to understand? Is there anything else about the Account Maintenance screens that you find confusing or would like to see improved? TASK 8: Locating Filters in Search Panels In the new offline app, you can open a filter panel to narrow your search results. The screenshot below shows an example of a filter panel from the Facility Search screen. The specific fields and field names may vary depending on your state’s configuration. These panels may contain many filter options—such as date ranges, locations, classifications, and status fields: Which approach would help you find the filter you need fastest?(Required) Filters listed in alphabetical order Filters grouped by category (e.g., all address-related filters together) Most frequently used filters listed at the top I don’t have a strong preference Regardless of your answer above, how important is it that the offline app's filter arrangement matches the online app?(Required) Very important—consistency between apps helps me work efficiently Somewhat important Not important—this would not affect my workflow TASK 9: Switching Between Organizations (Supervisors Only) Users with supervisory access can switch between multiple organizations in the current disconnected app. We’re evaluating whether to include this capability in the new offline app. When you're working offline in the field, how important is it to access records from multiple organizations during a single session?(Required) Essential—I regularly need to switch between organizations while working offline Helpful but not critical—I could wait until I’m back online to access other organizations Not important—I typically only work within one organization while in the field I don’t currently use this feature General FeedbackWould you like to share any general feedback about today’s survey?
